If you pan down on this board you'll see a thread *engine vibration - Joe Koehler February 26, 1998* which includes a response from me on Mr. Koehler's problems.His problems sound similar to yours.
I read your description and you seemed to have covered everything except possibly crankshaft-transmission alignment.
Too bad we haven't heard back from Mr. Koehler on what he found or resolved.Perhaps you could email him directly and get a two way discussion going.
