welding crankshaft journals

I know of a few people that have had journals built up by welding. The process starts with undercutting the journal on a crank grinding machine. The original radius at each edge of the journal are left intact (stress point in a sharp corner).
Next the journal is welded up on a journal welding machine. Then back to the crank grinder to be sized. No heat treat to my knowledge. These cranks are crack tested before they are welded. Later these cranks are breaking at the welded journals. Why are they breaking.
