Dean,
This is a common problem and there are many solutions.If the outlet neck broke right across the hole where the stud fits then it is a gasket problem.You need to have enough gasket on the outside part of the neck to keep it from breaking.Some people use a couple of broken off paper matches inserted on each side (front and back) of the bolts. Also make sure that the mating surfaces of the neck and head are both flat and true all the way to the edge.I am sure that others will have ideas. Hang in there.
