I agree that "wallered-out" holes in the wheels can cause the lug nuts to bottom out on the drum or stud and not tighten the wheel properly, causing movement and loosening of the nut or further wear of the holes.
Another common problem is worn stud threads and worn or improper reproduction lug nuts.Some nuts don't have enough relief of the threads near the outside to allow them to be tightened properly, and some just don't have the prop[er threads or inside length. Don't use locktite - the nuts should be installed dry and torqued to 40-50 ft.lbs.Try good-quality reproduction nuts first, then replace the studs if still a problem.
